A request to change your name in the Eastern Washington University's student information system may be made either in person or in writing by printing and completing the Name Change form and submitting it to the university.

In order to make the name change you will need to supply documentation to verify the change. The documentation may be a copy of a marriage certificate, divorce decree, court order or 2 pieces of identification establishing you are doing business under the new name. (one of these pieces of ID must be picture ID, such as a driver's license. Eastern's student identification card will not work as one of the pieces of identification). A court order is required for a total name change.

Name changes may be done in the Registrar's Office located at 201 Sutton Hall in Cheney or by submitting the name change form to the Registrar's Office, Eastern Washington University, 201 Sutton Hall, Cheney, WA 99004-2448.
