Faculty Digital Badge Award Request


Form Instructions

This form uses conditional logic based on whether you are submitting for bulk award requests or single award requests. If you would prefer to submit single student requests, you are welcome to do so as many times as needed.


1. Select Request Type
  • Single Student Award – submit for one student at a time
  • Bulk Student Award – submit for multiple students using a template
2. Select Award Type
  • Course Badge
  • Certificate Badge
3. Complete Required Fields
  • Follow the prompts—only relevant fields will appear based on your selections.
4. Course Substitutions (Certificate Awards Only)
  • Single Student Request: You will be asked to confirm any course substitutions directly in the form.
  • Bulk Request:
    • Download the provided template
    • Enter student information and any course substitutions inside the template
    • Upload the completed file in the form
5. Submit the Form
  • Review for accuracy before submitting to avoid processing delays.

Updating a Badge

If a badge is submitted and approved and has gone through substantive change, please submit a new EWU Parchment Digital Badge Request Form with these changes. Upon approval, the original badge will be archived and student issued badges in EWU Parchment will no longer be visible or issued through the EWU Issuing department page. The new approved digital badge will be made available to all students going forward. This will not affect or negate the validity or earning criteria of historically issued badges under the original version in EWU Parchment. These students will still have access to share their badges upon archiving from EWU Parchment system.
