DUTIES & RESPONSIBILITIES
1. Provides physical therapy services to patients according to a written physician’s plan of care. This may include, but will not be limited to:
A. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
B. Assesses for muscle strength, mobility, gait, ROM—potential for rehab.
C. Directing physical therapy treatment.
D. Instructing patients and families/caregivers in the use and care of therapeutic appliances.
E. Determining priority needs for physical therapy
F. Reporting to physician patient’s reaction to treatment or changes in condition.
2. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
3. May train patient in the use of prosthetic device.
4. Identifies patient and family/caregiver needs for other home health services and refers
5. as necessary.
6. Prepares and submits clinical and progress summaries based on the attainment of goals.
7. Participates in discharge planning for patient.
8. Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
9. Provides physical therapy consultation to home families/caregivers when indicated.
10. Provides in-service education programs for nursing organization personnel as needed.
11. Participates in peer consultation process.
12. Supervises Physical Therapy Assistants according to organization policy and state regulations.
13. When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.

Supervises and acts as a clinical resource to Golden Bear Physical Therapy and Sports Injury Center, Inc. staff, both clinical and administrative.
Monitor schedule for new patients on appropriate PT schedules for evaluations.
Work with RDO, SMA, and PCCs to maintain evaluation load and schedule compliance to meet company standards and requirements.
Generation of statistical reports, Analysis, and data synthesis of provided statistics in dashboard, development, and implementation of plans based on data synthesis.
Daily monitoring of patient visits, and cancellations, and analysis of daily, weekly, monthly trends.

Monitor all patient discharges in alignment with Company standards of care.
Ensure every patient receives a thorough, comprehensive, and customized physical therapy regimen, while receiving a unique experience.
Provide a written and hands-on evaluation and immediate treatment of incoming patient conditions to determine appropriate physical treatment programs.
Ongoing scheduling of patients to ensure fluid treatment progression.
Develop and implement Physical Therapy treatment programs based on each patient’s particular needs.
Assist patients with all phases of Physical Therapy treatment and techniques to include:
exercise instruction (both in-clinic and take-home instruction)
manual therapy techniques
functional/total body progressions
Discuss treatment with patients and record patient conditions and responses to treatments in the patient’s medical record after each visit.
Complete daily patient care notes, progress notes, discharge notes, and other required forms as requested by treating physicians, rehab nurses or insurance companies.
Ensure daily treatment notes for all patients are complete prior to end of day.
Provide continuous communication with referring physicians and Physical Therapy Assistants regarding re-evaluation findings to continually modify and update treatment programs.
Accurately document treatments rendered in the appropriate patient’s medical record.
Ensure billing amounts on daily notes match treatment time and that all units of treatment are accounted for.
Ensure billing is accurate and complete prior to signing off on the daily note in accordance with Company policy, state practice act, and federal regulations, including but not limited to HIPAA, Medicare and Medicaid.
Review and co-sign notes of all patients treated by auxiliary personnel under PT’s direct supervision.
Ensure all patients are discharged with a positive outcome and experience or have been proactively referred to the next step to address limitations and/or complaints originally presented.
Follow-up with discharged patients, maintaining Company’s service standards.

Eastern Washington University is sponsoring a Multidisciplinary Health Career Fair on Thursday, April 15, 2021, from 3:00 pm-6:00 pm. The career fair will be held virtually due to the pandemic.

Students are very eager to meet prospective employers and we encourage you to attend the Career Fair to meet them. Students that will be attending are in Physical Therapy, Occupational Therapy and Speech Language Pathology, University Programs in Communication Disorders, EWU and WSU.

Since the Career Fair is virtual this year, we are reducing the fee. The registration fee is $200.00 if payment is received by March 26, 2021, and $250.00 if received by April 2, 2021.
